If you or a loved one has been diagnosed with a chronic disease such as heart disease, high blood pressure, high cholesterol, stroke, lupus, arthritis, diabetes, COPD, Crohn’s or ulcerative colitis, this program can help you take control of your condition by offering resources, support and health education at no-cost to you.  Topics include but are not limited to: symptoms and side effects, diet and exercise tips, pain/fatigue management, medication management, and management of stress and emotions. 

Participants meet for two hours, once a week, for six weeks.
